Introduction
Mosslands Medical Practice is a GP partnership providing General Medical Services to the registered population of Irlam and Cadishead. The practice is committed to delivering safe, effective, compassionate and high-quality primary care services that meet the needs of our patients and local community.
The practice is led by a team of three GP partners, supported by a multidisciplinary team of clinical and administrative staff. We work collaboratively to provide accessible healthcare services while ensuring that patients receive care that is respectful, personalised, and responsive to their individual needs.
Our aim is to enhance patient care and experience by delivering services that are clinically effective, well-managed, and centred around the principles of dignity, compassion, and partnership.

Services we provide
Mosslands Medical Practice provides a comprehensive range of primary care services to our registered patient population of approximately 12,500 patients.
The practice operates a clinical triage model, designed to ensure patients receive the most appropriate care from the most suitable healthcare professional on first contact.
Clinical requests are reviewed through GP-led triage, with urgent clinical matters assessed during the morning triage process and supported by an on-call GP service during the afternoon.
The practice works collaboratively with other healthcare providers to ensure patients receive coordinated and appropriate care.

Mosslands Medical Practice is registered with the Care Quality Commission (CQC) to carry out the following regulated activities:
Treatment of Disease, Disorder or Injury
The practice provides assessment, diagnosis, treatment, monitoring, and management of a wide range of acute and long-term health conditions. This includes general medical consultations, chronic disease management, preventative healthcare, minor illness management, and treatment provided by appropriately qualified healthcare professionals.
Diagnostic and Screening Procedures
The practice provides diagnostic and screening services to support early identification, monitoring, and management of health conditions. This includes cervical screening, routine health checks, long-term condition monitoring, and other clinical investigations undertaken within primary care.
Surgical procedures
The practice provides minor surgical procedures carried out by appropriately trained and competent clinicians. These services may include minor skin procedures and joint injections,
Working with our Primary Care Network
Mosslands Medical Practice is a member of Eccles & Irlam Primary Care Network (PCN). Through this partnership, the practice benefits from additional healthcare roles and services that support patient care, including pharmacy services, mental health support, the ACT service, and urgent care services.
Working within the PCN enables the practice to provide a wider range of support to patients and improves access to appropriate healthcare professionals.

The practice was inspected by the Care Quality Commission (CQC) in July 2023 and was rated as Good.
